Continuing and Complex Care

  • Hospital Admission Risk Program (HARP) - service which supports people in the community to manage their Chronic Illness and assist in the prevention of hospital presentations or admissions
  • Residential In Reach (RIR) – provides residential aged care facilities, an on-site alternative to presenting to emergency department for a range of common presentations where it is safe to do so.
  • Post Acute Care (PAC) – up to 28 day services to support early discharge
  • Restorative Care Program (RCP) – slow rehab program to transition back to home or aged care facility

  • Transitional Care Program (TCP) –  client fees applicable– ACASS assessment required

Continence

  • Urodynamics – assessment of bladder function
  • HACC & Subacute Continence Nurse Advisors
  • Continence Childrens Disability Therapy – mainly provides children with disability assessment and packages for continence issues
